WHAT WE OFFER :

 

Health Benefit Plan Individual employees are covered at marginal cost.  Family Coverage available on a pre-tax basis if you elect to participate in the Section 125 Plan.

 

Life Insurance Toccoa Clinic provides a $15,000 life insurance policy for all employees at no cost to the employee.  Dependent coverage is available if you have the dependent health benefit and is included in the dependent health premium.

 

401(k) and Profit Sharing Plan Eligibility semi-annually following one year and 1000 hours of service. 

 

Voluntary Plans – Toccoa Clinic also has voluntary plans available.  You pay 100% for the cost of the coverage.  You may elect to either participate or not participate in the following plans:

 

Dental  Fully insured dental plan with 4 pricing tiers available on a pre-tax basis if you elect to participate in the Section 125 Plan.

 

Accidental Death Dismemberment Rates are based on amount of coverage requested as outlined in the plan.

 

Standard Life Rates are based on age and amount of coverage requested as outlined in the plan.

 

Supplemental Plans: Cancer, Accident, Recovery, and Hospital policies available on a pre-tax basis if you elect to participate in the Section 125 Plan.

Short-Term Disability and long term care also available.

 

Additional benefits
